Job Summary : Responsible for day-to-day supervision of Personal Care Workers and client care and services, including but not limited to, managing caregiver schedules, overseeing client admissions, supervision and direction of caregivers, maintaining client files, Plan of Care, and high-volume phone calls.

Qualifications : Be at least eighteen (18) years of age.Have appropriate experience or training in the home care industry or closely related personal care services in accordance with HCA policy.

Have completed training in the provision of personal care services.Previous IHSS, EBD or Case Management experience preferred.Responsibilities :

• Supervision of caregivers per HCA policy
• Ensure caregivers have been trained and prepared for assignments
• Outstanding communication skills (Verbal / Written)
• Exceptional client service
• Organizational skills and completion of assignments in a timely manner
• Provide client assignments to caregivers
• Follow up on client grievances and provide resolution
• Coordinate client services with external health care entities
• Excellent customer services
• Maintain high visibility and availability via telephone and email.
• Perform tasks as delegated by HCA Administrator.
• Actively develop and pursue referral sources, as well as facilitate on-going relationships with various community organizations.
