The Institute on Aging is looking for a dynamic Program Manager to join our team! We are looking for a leader that is a quick learner, compassionate, adaptable, flexible and ready to oversee one of the fastest growing programs at the agency.
The primary responsibility is to manage Program Operations for Community Living Fund, including the Clinical Supervisors, Occupational Therapist, Care Managers and Administrative Staff.
ESSENTIAL JOB FUNCTIONS:
- Manage vendor and community agency partnerships.
- Monitor program waitlist, caseload sizes, and ensure timely assignment of new cases in order to fulfill contract requirements.
- Ensure CLF Program Policies and Procedures are consistent with IOA Policies and Procedures.
- Implement collective bargaining agreement changes and ensure program compliance.
- Revise CLF Program Policies and Procedures annually with CLF Leadership Team.
- Clearly define roles and responsibilities of all program staff including identifying areas for professional growth and development.
- Ensure all CLF program staff receive performance appraisals at least annually and after the first six months for new staff.
- Develop and implement nursing home transitions program in collaboration with Medi-Cal Managed Care health plans and the IOA San Francisco implementation team.
- Direct an interdisciplinary, multi-agency team of clinical experts in San Francisco who are responsible for transitioning and supporting individuals out of LHH into more independent settings.
- Work in collaboration with San Francisco County providers to identify gaps in community-based services and supports for high-risk, low-income individuals.
- Ensure CLF documentation and clinical records are in compliance with program policies & procedures, contract guidelines, and meet the expectations of internal and external audits/reviews.
- Participate in recruiting, onboarding, performance management, terminating of all CLF staff.
- Participate in relevant Department of Aging and Adult Services (DAS) meetings and community workgroups that are relevant to CLF.
- Participate in relevant meetings and workgroups within Medi-Cal Managed Care plans and County providers to develop a more integrated long-term care community support system.
- Develop infrastructure for referrals to housing partner and regularly report to DAS any barriers to timely housing for LHH transitions.
- Become an expert in supportive housing options in San Francisco; integrating Community Living Services into new and existing housing resources.
- Participate in local and statewide associations advocating on behalf of improved community based long-term care services.
- Modify CLF Annual Plan with DAS program analyst and DAS program manager.
- Participate in DAS 6 month report creation with DAS staff, including data review with DAS Analyst.
- Collaborate with Senior Leadership to identify additional funding opportunities to support the expansion of services in San Francisco.
- Work with other IOA Senior Managers, Senior Directors and VP of Community Living Services to develop best practices for community-based managed long-term support and services.
- Work with Clinical Supervisors to develop short term and long-term programmatic goals and objectives.
- Develop internal and external budgets, forecasts, and variance analyses.
- Participate in relevant Institute on Aging meetings and work groups to integrate program operations into the agency.
- In collaboration with IOA Marketing Department, develop and present materials and outreach information to interested stakeholders in the community about IOA.
- In collaboration with Senior Director, identify opportunities to streamline administration of regional programs intended to support seniors and people with disabilities living in the community.
- Administratively support operations and develop relationships with new funding sources.
- Attend DAS Commission to represent CLF and respond to Commissioner questions.
- Participate in CalAIM San Francisco planning with Sr Director of Transitions, VP of Central Operations, and office of Strategic Partnerships.
- Improve CLF data collection to align with Division goals.
- Evaluate Acuity Tool usage, accuracy, and consistency with Division; modify practices as needed to align.
- Develop improvement plans to ensure all contract deliverables are achieved annually.
- All other related duties as assigned.
BACKGROUND & EXPERIENCE:
- Minimum 2 years supervisory experience in medical or community-based setting.
- Demonstrated understanding of community-based resources and long term care supports and services.
- Ability to develop and successfully manage large numbers of professional relationships.
- Understanding of community-based case management preferred.
- Experience in building and leading an efficient and effective multi-disciplinary team preferred.
EDUCATION REQUIREMENT: Master’s degree in administration, public health, social work or a related field (nursing, psychology, counseling, rehabilitation, gerontology, or sociology) from an accredited school.
PREFERRED: Licensed Clinical Social Worker, Licensed Psychologist, or Registered Nurse
OTHER DESIRABLE QUALITIES:
- Demonstrated ability to work collaboratively and to encourage collegial working relationships and team building within agency and with outside partnerships.
- Excellent verbal and written communication skills.
- Broad understanding of community-based health and social services and pertinent policy issues.
- Demonstrated ability to identify and develop new funding streams and partners.

COMPENSATION:
- Range: $ 105,289.00 - 142,250.00/Annual
This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.
This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
