Social Worker - MSW - Care Services (Admissions)
MSW required - LCSW highly preferred. Must be licensure eligible. One year of Social Work experience in a Healthcare setting required. The candidate must have excellent patient/family communication and listening skills as the candidate will be guiding/supporting the family and advocate for their care choices as they are making difficult decisions while in the inpatient setting. The candidate will collaborate with hospital staff and physicians and assure coordination/implementation within the hospital of the hospice plan of care. The candidate will provide documentation to the hospital of the patient plan of care and actively coordinate the hospice patient discharge from the hospital. Computer skills & experience required. Monday thru Friday - Days with some early evenings and weekend rotation.
