Pediatric Social Worker

HealthCare Support is actively seeking a Pediatric Social Worker to fill an opening with a Fortune 50 Managed Care in Lee County FL - seeking candidates located in Ft. Myers.

Daily Responsibilities for Pediatric Social Worker:

• Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients
• Provide patient and provider education
• Facilitate member access to community based services
• Monitor referrals made to community-based organizations, medical care and other services to support the members’ overall care management plan
• Monday to Friday, 8am to 5pm (No OT required)/Hybrid
• Role starts out on 3 month contract with potential for permanent hire.

Required Qualifications for Pediatric Social Worker:

• Must have a MSW and 1 year of Pediatric experience OR LPN with 4 years of Pediatric experience
• Must have a Valid and Active Driver’s license and current vehicle insurance
• Must have MS Office experience - Excel, Outlook, Word

Preferred Qualifications for Pediatric Social Worker:

• 2 years of pediatric experience or 1 year pediatric and 1 year general social work experience
• Experience with EMR
• Case management
• Bilingual in Spanish highly preferred

Pay Details: $27 - $32.69 an hour
