Social Worker (LGSW, LCSW, or LICSW accepted) - Washington, DC

Pay range: $71,260 - $100K 

Full-time

Position Summary:

The Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the patient’s plan of care from admission to post-discharge. 

Education/Qualification Requirements:

• Prospective employees can be licensed as a LGSW, LCSW, LICSW in the District of Columbia (For LGSW we offer supervision)
• Master's degree in Social Work
• 1-3 years' experience in acute care or other related healthcare field is preferred
• Case Management Certification preferred.

Responsibilities:

• Utilize critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient’s needs and ensures a seamless, effective, and efficient transition of care across the continuum. Coordinate, negotiate, manage, and implement the care of complex patients to facilitate achievement of quality and cost-effective patient outcomes.
• Work collaboratively with interdisciplinary staff internal and external to the organization.
• Participate in quality improvement and evaluation processes related to the management of care.
