- Licensed master’s level social worker with a minimum of 2 years’ experience providing case management to veterans with a HUD VASH voucher.
- A valid driver’s license.
- Conduct initial screenings and assessments to determine the eligibility and suitability of Veterans for the HUD-VASH program.
- Perform thorough evaluations to understand the individual needs, challenges, and goals of each Veteran to tailor case management services accordingly.
- Connect Veterans with necessary medical, mental health, and social services either individually or through group sessions.
- Assist Veterans in navigating the process of obtaining housing vouchers from Public Housing Authorities.
- Conduct regular home visits to monitor housing stability, assess social well-being, and facilitate access to necessary resources and support services.
- Offer financial management and skills development training to enhance Veterans' abilities to maintain stable housing and self-sufficiency.
