The Institute for Family Connections™ is currently hiring for a Permanency Specialist II who will be working with youth and families receiving services from Monroe County Department of Health and Human Services at our Rochester, NY location.

This is a Full Time position - 40 hours per week. Hours will vary throughout the week.

These youths will be at risk of entering foster care or may be currently residing in foster care. This is a unique collaboration between Hillside’s Preventive and Permanency Services, where extended relatives and fictive kin will be sought and gathered by a Permanency Specialist, and a carefully planned transfer will occur to a General Prevent Clinician, who will continue providing services to the youth and family. The Permanency Specialist is responsible for individualized permanency planning, which includes the delivery of intensive short-term Family Finding services. The person in this position will have continuous collaboration with Monroe County DHHS, community partners, clinicians, care coordinators and other providers, in order to urgently locate and engage family members and other significant adults, to build a lifetime network of supportive, caring adults for each youth and family.

Responsibilities include, but are not limited to:

• Assessing and addressing the permanency needs of children/youth, and/or partnering with families who want to provide permanency for children/youth.
• Increasing supports for youth and families enrolled in services within the Institute for Family Connections (IFC). For youth, this entails creating a lifetime network of support by locating and engaging family members and other significant adults to provide on-going support for the youth to achieve positive and successful permanency outcomes.
• Creating a strength-based permanency plan that includes home time and assessments, identifying and coordinating potential permanency resources and other natural supports, recognizing gaps and needs, assessing results and providing on-going communication with the youth’s treatment team.
• Actively participating in agency Permanency Roundtables and serve as a permanency consultant to multidisciplinary treatment teams.

Requirements: Bachelor’s degree in a Human Services related area of study is required. An equivalent combination of education and experience will be considered for meeting the qualifications/requirements of the position. All candidates must have a valid NYS Driver’s License and must meet agency driving and insurance standards.

$18.36 Minimum Rate per hour, $27.54 Maximum Rate per hour, based on experience.
