Heartland Center for Behavioral Change is Hiring a Community Support Specialist Near Branson, MO

GENERAL DESCRIPTION

This is a responsible position providing a community based, healthcare provider of substance abuse, mental health, and co-occurring disorder treatment services. This position provides services with or on behalf of a particular client in accordance with his/her individual needs to maximize the client's adjustment and functioning within the community to achieve sobriety and sustained recovery, maximizing the involvement of natural support systems, and promoting client independence and responsibility.

ESSENTIAL FUNCTIONS

Work may include, but is not limited to, the following examples:

  1. Providing holistic, person centered care with emphasis on person strengths, skills acquisition and harm reduction, while using stage-wise and motivational approaches that promote active participation by the individual in decision-making and self-advocacy in all aspects of services and recovery/resiliency.
  2. Insuring all treatment plans are updated continually by referencing the client record
  3. Insuring all services are made available to each client based on the client need as outlined in the treatment plan.
  4. Facilitating and supporting recovery/resiliency through activities including: defining recovery/resiliency concepts in order to develop and attain recovery/resiliency goals; identifying needs, strengths, skills, resources and support then teaching how to use them; and identifying barriers to recovery/resiliency and finding ways to overcome them.
  5. Developing, implementing, updating, and revising as needed, a care plan that identifies specific, measurable and individualized interventions to reduce and manage symptoms, improve functioning and develop stability and independence. This plan is developed by a team of the appropriate supports.
  6. Initiating and maintaining contacts as required by policy with referral and third-party payer sources.
  7. Providing services that result in positive outcomes.
  8. Documenting services that clearly describes the need for services, the intervention provided, and the relationship to the care plan, and the provider of the service. The date, actual time, and setting of the service, and then individual response to the service.
  9. Developing a discharge and aftercare/continuing recovery plan to include, if applicable securing a successful transition to continued services.
  10. Provide information and education to consumers and their families in order to learn about and manage mental illness/serious emotional disturbance and/or substance use disorder.
  11. Building skills for effective illness self-management, including psychoeducation, behavioral tailoring for medication adherence, wellness/recovery planning, coping skills training, and social skills training.
  12.  Helping individuals develop skills and resources to address symptoms that interfere with seeking or successfully maintaining a job.
  13. Demonstrating and modeling professionalism in order to achieve the highest standards of services.
  14. May transport consumers to services and therapeutic activities, as required.
  15. Performs other related work, as assigned

EXAMPLES OF KNOWLEDGE, SKILLS, AND ABILITIES

  • Basic knowledge of substance use disorders and co-occurring addictions and mental illness diagnoses
  • Basic knowledge and understanding of human behavior, attitudes, and motivation
  • Excellent written and verbal communication skills
  • Skilled in developing positive and productive participant relationships
  • Ability to negotiate complex service systems to obtain needed services and resources for participants while not making decisions or performing activities that the participant is fully capable of performing
  • Ability to develop and maintain accurate, legible, organized records, and to prepare timely reports using mental health support systems such as CIMOR or CARELOGIC
  • Ability to plan and organize working time effectively and to meet deadlines
  • Ability to work harmoniously and professionally with state and local officials, employees, consumers, consumer families, guardians, public agencies, and the general public
  • Ability to work effectively with persons from diverse cultures and backgrounds and to respect individual differences
  • Ability to collect and analyze information in making decisions concerning a consumer's treatment
  • Ability to handle confidential information appropriately
  • Ability to recognize potential ethical problems and address in ethical manner
  • Ability to work with autonomy or with minimum direction
  • Skill in resolving conflicts and negotiating with others
  • Working knowledge of current resources available to persons with substance use disorders
  • Basic knowledge of the healthcare system

EXPERIENCE AND EDUCATION QUALIFICATIONS

  • Bachelor's Degree from an accredited college of university in social work, psychology, nursing or closely related field
  • OR
  •  3 years of qualifying experience in related field

Qualifying experience must include some combination of the following:

  • Providing one-on-one or group services with a rehabilitation and recovery/resiliency focus;
  • Teaching and modeling for individuals how to cope and manage psychiatric, developmental or substance use issues while encouraging the use of natural resources;
  • Supporting efforts to find and maintain employment for individuals and/or to function appropriately in families, school and communities;
  • Assisted individuals to achieve the goals and objectives on their individualized treatment or person centered plans.

NECESSARY SPECIAL QUALIFICATIONS

No candidate may be considered for employment who is, or has been for the past two (2) years under the supervision of Federal, State, County or Municipal Corrections Department nor have any pending felony or misdemeanor charges other than minor traffic charges.

Driving may be required for this position. The successful candidate must have and remain in possession of a valid current driver's license, automobile insurance, and be insurable under the agency's motor vehicle insurance policy.

Must pass a background check provided for the agency by the U.S Department of Justice and the Family Care Safety Registry. Must not be on any "do not hire" list with the Department of Mental Health or the Missouri Division of Family Services.

All candidates for employment must meet the established minimum qualifications for employment as identified by the applicable regulatory agencies.

Current certification in CPR and First Aid or achieves certification within 60 days of employment. (Note: This certification must be maintained current throughout employment).
