Community Counseling Center is Hiring a Behavioral Health Assessor (QMHP) Near Fredericktown, MO

Behavioral Health Assessor (QMHP)


Location: Fredericktown, MO & Marble Hill, MO
("home base" would be dependent upon agency need and candidate location)

Pay Range: $42,000 - 47,000/year - dependent upon relevant experiences, credentials, etc.

Full-Time: Monday - Friday: 8a - 4:30p

Minimum Required Qualifications:

  • Must meet the requirements of a Qualified Mental Health Professional
  • Must have a valid driver's license for current State of residence, meet agency MVR standards, and must provide and maintain continuous auto insurance.

The CPRC Program
Community Psychiatric Rehabilitation Center (CPRC) is a program designed for adults and children with disabilities in social role functioning and daily living skills. Program staff work closely with clients in a variety of community settings to provide wide-range case management services and support in/through therapy to promote recovery.


Your Job as the Assessor

As an Assessor, your responsibility will be to complete the


Position Specific Essential Functions

  1. Complete initial, annual, and brief evaluations for clients enrolled to CPRC services as assigned.
  2. Complete assessments in client's home or other location when needed to accommodate the client.
  3. Complete assessments within the timeframe designated by CCC.
  4. Use clinically appropriate language in assessments and follow agency guidelines regarding content of assessments.
  5. Develop/revise individualized treatment plans for each client following agency guidelines.
  6. Add required signers to the assessment/treatment plan and let signers know the document is ready for their signature.
  7. Ensure coordination of care by communicating concerns regarding client's mental health status or treatment with CSC and CPRC supervisor.
  8. Assist in screening clients for Substance Use Disorders and provide diagnosis if clinically appropriate as needed
  9. Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
  10. Assist in completing phone survey and eligibility determinations as needed
