We are presently seeking full-time Community Health Navigators.
Community Health Navigators enhance the quality and utilization of HIV services by providing counseling, outreach, and referral services for patients and their support systems.
Must have a high school diploma, preferably with at least one to two years’ experience working with HIV or OB patients. Experience in integrated health services, social determinants of health assessment, outreach, caseload management and/or tracking is a plus. Candidates should be able to travel to multiple sites throughout GPHA's healthcare network and work proactively both independently and as part of a team.
