Primary Function:The community health specialist will work on a team to support patients with complex medical and social needs to navigate the health care system, connect with community services, and adopt health behaviors. Key activities will include building trusting relationships, assessing individual needs, and providing ongoing support. The role will engage with patients in the home, in the community, and at health care facilities where the patientrsquo;s receive care. The overall goal of the program is to keep patients as healthy as possible and out of the hospital.
Education/Training/Experience:
middot; High School Diploma or equivalent required. Associates Degree and/or Community Health Worker certification preferred.
middot; At least 2 yearsrsquo; experience in healthcare field, community-based setting and/or completion of Community Health Worker formal training program such as from a college or other education institution. Consideration will be given to experience caring for someone with a significant health need.
middot; Ability to travel efficiently to multiple locations throughout the work day required.
middot; Basic computer skills required
middot; Ability to initiate and maintain positive working relationships with staff, primary care and specialty practices, health plans, and community organizations required
middot; Committed to working in and supporting members of the community required. Strong communication skills, including, listening, using appropriate language, and writing/typing skills required
middot; Demonstrated experience providing emotional support, encouragement, and motivation to people with complex medical and/or social challenges required
middot; Experience and knowledge of healthcare insurance preferred
middot; Knowledge of medical terminology preferred
