The Community Health Worker (CHW) will assist pregnant and parenting individuals in treatment for substance use disorder and their children toward healthy recovery behaviors. The CHW will help women engage in treatment and their families navigate and access community resources and services. The CHW will provide support to the clinical and medical teams: care coordinators, navigators, nurses and therapists through a team based care model. The CHW will provide social support, coaching, and education to support women’s active participation in treatment goals and objectives. The CHW will support the treatment teams to help women access and attend treatment, community resources and services, and advocate for individual and community health needs. The CHW will also write process notes, collect data, and complete required documentation.

ESSENTIAL FUNCTIONS:

  • Assists in facilitating treatment team directives for women and their children. This includes working with women and their families or kinship networks. Identifies and help enroll eligible individuals in federal, state, and local private or nonprofit health and human services programs
  • Assists and/or accompanies women with healthcare, social services, court appearances and other professional appointments for women and their children.
  • Serves as a liaison between women/families/communities and other treatment and healthcare agencies
  • Provides Guidance and social assistance to patients families and community residents
  • Enhances patients’ ability to effectively communicate with the substance use treatment teams and healthcare providers
  • Provides culturally and linguistically appropriate health education materials within the scope of training
  • Provides referral and follow-up services
  • Assists women in treatment and their families in understanding and carrying out their treatment team and healthcare providers’ advice
  • Follows-up to assess how the women are recovering and adjusting well to their treatment plans, providing additional support as necessary.
  • Compiles and tracks all interactions, process notes and other documentation as needed.
  • Front desk duties as assigned such as greeting guests and patients’ families, checking in women and coordination of care,
  • Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.

EDUCATIONAL/TRAINING REQUIREMENTS:

High School Diploma or GED.

CERTIFICATES, LICENSES, AND REGISTRATION:

Certified Community Health Worker training program required and Community Health Worker certification completion or in process.

EXPERIENCE REQUIREMENTS:

Prior experience in community outreach preferred.
