AbsoluteCare offers concierge health services using a risk-bearing, PCP-driven care model. We treat the most clinically complex and vulnerable members of the communities we serve, many of whom suffer from behavioral health, substance use, and SDoH challenges. The Community Health Worker- BH works to locate, engage, and educate members referred to AbsoluteCare by their health plan. Establish trust and build meaningful relationships with members. Promote AbsoluteCare medical center as the “Go-To” resource for our members instead of using the emergency room or other fragmented higher cost medical resources. This role helps improve access to health care services, increasing health and screening, enhancing communication between community members and health providers, and reducing the need for emergency and specialty services. Locate, engage, and educate member on AbsoluteCare’s services. Explain AbsoluteCare’s model of care and the benefits to the member if choosing AbsoluteCare as primary care provider (PCP). Provide home visits to members.

Education and Experience

• Must have a valid driver’s license and reliable personal vehicle.
• Previous experience managing a team.
• Proficient in PC skills, such as Microsoft Office.
• High school diploma or equivalent required, Bachelor’s degree preferred.
• Community Health Worker experience/certification desirable.
• Ability to work evening and weekend hours when needed.
