Granite VNA is Hiring a Home Care Manager Near Laconia, NH

Description

Monday - Friday 8:00 a.m. - 4:30 p.m. 

Reporting to the VP of Home Care, the Home Care Manager is responsible for clinical business operations and patient care services of the agency’s Home Care Services which includes planning, organizing, developing and managing the agency’s Home Health Care Services in accordance with federal, state and local regulations, standards, and guidelines as well as agency policies and procedures. Provides mentorship and guidance of clinical guidelines to the field staff professional and paraprofessional as needed. Performs all functions in a professional and ethical manner, and collaborates with other members of the health care team to ensure the provision of quality patient care. Directs and supervises the activities of subordinates and works under the interdisciplinary team model. The Home Care Manager supervises Registered Nurses (RN), Licensed Practical Nurses (LPN), Physical Therapists (PT), Physical Therapy Assistants (PTA), Occupational Therapists (OT), Occupational Therapy Assistants (COTA), Speech Language Pathologists (SLP) and Medical Social Workers (MSW). 

Essential Functions

  • Home Care Manager is responsible for oversight of the daily operations in the Electronic Medical Record (EMR). The Home Care Manager may delegate this responsibility in his/her absence to a qualified Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Speech Language Pathologist (SLP) or Medical Social Worker (MSW). 
  • Provides clinical supervision oversight to all personnel in the Home Care program. RN and LPNs may receive clinical support from the Home Care Manager as needed.
  • Coordinates patient referrals and patient care, ensuring that patient needs are met and continuously assessed according to the individual needs of the patient and physician ordered Plan of Care. 
  • Develops standards and procedures for providing home health care and for evaluating service. Scope of services include, but are not limited to, Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists, Medical Social Workers and Speech Therapists. 
  • Assures compliance with all state, federal laws regarding licensure and certification of the agency. Has knowledge of the Medicare Conditions of Participation for Home Health and is knowledgeable of and understands Hospice Conditions of Participation.
  • Promotes an atmosphere of unity between professional staff, paraprofessional staff support staff and clients/families. Seeks to diminish conflict by creating an environment that promotes trust and open communication.
  • Oversees and makes changes as needed with the assignment of care to appropriate discipline based on the patient’s needs. 
  • Organize and complete individual case conferences with staff assigned to their team every 2-4 weeks. Participate in the coordination of staff meetings. The meetings will occur on a consistent basis and have a defined schedule with an agenda. 
  • Facilitates weekly IDT meetings with staff to ensure that the plans of care, goals and discharge plans for multi-disciplinary cases are aligned and coordinated between team members.
  • Monitors clinical documentation to ensure compliance with agency policy and procedures, regulatory and accreditation requirements. These include: ensures that the plan of care is being followed, that the appropriate interventions are being addressed, that the patient/family is progressing appropriately, that the documentation of the education provided and the communication with other providers is evident.
  • Has a working knowledge of third-party contract requirements in relation to clinical summary of care requirements necessary for authorization for care. 
  • Coordinates field supervisory visits of staff assigned to their team in accordance with agency policy and procedure. Utilize appropriate discretion in regards to increasing field supervision when performance issues arise. 
  • Completes performance evaluations for staff at the end of the introductory period and annually. 
  • Reviews management statistical reports directly related to the teams’ performance. Communicate trends to staff as appropriate. 
  • Supervises and monitor performance of assigned staff for compliance with Agency personnel policies. 
  • Establishes priorities and attainable goals for keeping daily operations functioning, based on program needs and the Mission and services of the agency. 
  • Interviews and recommends new staff for hire for the Home Care Program.
  • Participates in the coordination and assists with orientation of staff to include teaching and documentation requirements, and to improve quality and quantity of services. 
  • Identifies gaps in services and coverage and makes recommendations to the Chief Operating Officer. 
  • Collaborates with Directors/Managers to deliver care in a coordinated manner for all clinical programs. 
  • Assures staff productivity standards are maintained. 
  • Works with and supports families, community agencies, and medical personnel to arrange for convalescent and rehabilitative care of home care patients.

Other Job Functions 

  •  Assists the VP of Operations in the development, revision, and implementation of Program Policies and Procedures. 
  • Participates on Agency and Community Committees as necessary. Serves on Advisory Boards, committees and/or community groups. 
  • Actively participates in Quality Improvement (QAPI) and activities, and shares outcomes and correction plan with staff.
  • Receives and documents reports of incidents, complaints, and grievances according to Policy and Procedure. 
  • Coordinate professional staff meetings. Encourage and support staff in professional growth and development. Seek opportunities to educate other departments regarding clinical and/or professional issues. 
  • Participates in preparing budgets and operational plans related to home care services, and agency strategic initiatives. 
  • Stays informed regarding resources within and outside of the Agency to promote and educate the staff. 
  • Demonstrates flexibility in role by accepting other duties as assigned for which qualified. 
  • Serves as a member of the Agency’s Management Team and participates in the development and implementation of overall policies, programs, objectives, and goals. 
  • Represents the Agency in contacts with other similar agencies and trade associations, clients, and regulatory agencies. 
  • Actively participates in the implementation of the Agency’s Emergency Preparedness Plan. 
  • Participates in the Administrative on-call rotation schedule. 

Requirements

  1.  Graduate of an accredited school of Nursing, Physical Therapy or Occupational Therapy, accompanied by current New Hampshire license, or compact state license. 
  2. Bachelor’s in Nursing, Physical Therapy, Occupational Therapy; Masters in health-related field preferred 
  3. One to two years of clinical experience (community health or hospital practice)
  4. Two to four years of management experience
