Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City’s leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults through-out Manhattan and New York City. We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families. The Isaacs Center focuses primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville, and seek to deliver programs that are impactful, innovative, and intergenerational.
Program Description: Goddard Riverside has four permanent supportive housing residences containing 421 beds for formerly homeless adults, many with mental illness, chronic physical diseases, and aging in place. On-site services include case management, entitlement assistance, medical and psychiatric care, substance use counseling, work readiness training, a meals program, 24-hour/7 day per week front desk security, and group activities. In addition, Goddard is home to a network of Older Adult Centers and Naturally Occurring Retirement Communities (NORCs) serving over 3000 older adults each year with congregate meals, case management, education/recreation activities, and a food pantry.
Purpose of Position: The Social Worker will screen older adults 55 years and older across the aging portfolio for depression, anxiety, and alcohol/substance use. Referrals to onsite and community treatment will be provided within five business days. Treatment planning, coordination, and outcomes will be tracked. Follow-up with clients will be conducted monthly.
Schedule: Monday through Friday 35 hours per week. Contract ending June 30th 2024.
