Position Description
Under the supervision of the Managing Clinical Director, and in accordance with established policies and procedures, the Social Worker provides client assessments, including diagnosing, substance use counseling, mental health counseling, HIV education, and entitlement advocacy services to clients of the Adult Day Health Care Program (ADHC). The Social Worker will also facilitate mental health and harm reduction groups. This position functions in a primarily electronic environment and requires proficiency in ‘Windows” based technology. CASAC certification a plus.
Essential Job Functions
The following duties are mandatory requirements of the job:
Education and Certification
Masters Degree in social work with LMSW, LMHC preferred. New graduates who will sit for the licensing exam immediately upon graduation will be considered if they have appropriate mental health experience. Candidate with experience in providing services to persons with HIV/AIDS in a health care setting are preferred. Experience with group facilitation is required. Expertise in securing housing and Medicaid entitlements preferred.

Established providers refused to provide our clients the care they desperately need, like housing, primary care, mental health, testing, health education and more. We founded Harlem United out of necessity. Specifically serving people living with HIV/AIDS who are homeless and/or suffer from mental illness, substance use, or social stigma related to their sexuality, gender or HIV status, Harlem United is the last resort for medically-under served communities of color in Harlem. We are about to open our newest site which will serve the entire community as a healthcare clinic for everyone, located on 133rd St between Lenox Ave and Adam Clayton Powell Blvd.
