Job Summary:

The Care Manager provides comprehensive, integrated care management and coordination for health and behavioral health care to individuals with co-occurring chronic conditions.

Must Have Qualifications:

• Associates Degree in Health/Human Services
• 2 years experience with Case Management
• Experience working with recovery supports, community resources, housing, employment, & other professional / nonprofessional services
• Valid NYS Drivers’ License
• Great computer skills along with knowledge of Microsoft Office Suite
• Self starter and works well under minimal supervision
• Loves working within the community

Nice to Have Qualifications :

• Bachelors Degree in Health/Human Services, Masters or higher preferred.

Duties:

1. Responsible for, but not limited to comprehensive assessment, outreach and engagement, service and treatment linkages and coordination using evidence based practices and outcomes.

2. Comprehensive client centered Care planning development and management with the individual to assist in the integration of chemical dependency, mental health and medical services.

3. Coordination of care with Finger Lakes Community Health Providers and FLACRA clinic providers as well as ongoing coordination of care with other providers identified in the care plan.

4. Assessment of psychosocial supports and linkages with housing, employment, transportation, medical, dental and mental health care as well as community resources and care planning.

5. Build and use effective communications strategies among peers, medical staff, addiction and mental health providers and other community agencies through the use of electronic assisted devices including Telehealth and other interactive technology.

6. Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence.

7. On-Call duties as assigned for nights and weekends

Other Skills/Knowledge and Experience:

1. Demonstrates ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.

2. Knowledge of local behavioral health services and substance abuse agencies.

3. Ability to multi-task in a fast paced environment, have good problem solving skills, as well as excellent time management and organizational skills and the ability to remain calm in a crisis while providing crisis intervention.

4. Must be able to type 30 WPM.

5. Proficient in Internet navigation, Microsoft Office, Outlook, Word, and Excel.

6. Proficient in the use of electronic health records.

Job Type: Full-time

Pay: $18.50 - $20.00 per hour

Expected hours: 37.5 per week
