English Road Pediatrics is Hiring a Behavioral Health Care Manager Near Rochester, NY

GENERAL JOB DESCRIPTION

The Behavioral Health Care Manager (BHCM) is a core member of the collaborative care team, which includes the patient’s primary care provider and psychiatric consultant, as well as the larger pediatric primary care team. The BHCM is responsible for supporting and coordinating the behavioral health care of patients on an assigned caseload with the family, primary care provider, and (when appropriate) other mental health and community service providers.

As an integral member of the pediatric primary care team, the BHCM monitors and measures the impact of evidence-based behavioral health interventions; and regularly re-assesses the patient’s progress including symptom reduction, level of psychosocial functioning, and overall well-being.

JOB DUTIES & RESPONSIBILITIES

Clinical Care

· Screen and assess patients for common behavioral health and substance abuse disorders. Provide psychoeducation and treatment options as indicated.

· Conduct initial and regular subsequent assessments of the patient’s behavioral health condition, needs, preferences, and barriers (including social determinants of health concerns).

· Provide brief behavioral interventions to patients and families using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, psychiatric medication adherence, and other treatments as appropriate. Brief interventions may be conducted by phone, video, or in person.

· Collaborate with family (e.g., parents, guardians, caregivers), school (e.g., counselors, social workers, teachers), medical teams (primary and specialty care), and community agencies (e.g., Child Protective, Foster Care, and other DSS caseworkers; probation officers) regarding patient behavior health needs.

· Develop a relapse prevention self-management plan with patients (and their families) who have achieved their treatment goals and are soon to be discharged from the BHCM caseload.

Care Management / Coordination

· Participate in weekly systematic case review (SCR) with the psychiatric consultant and communicate resulting treatment recommendations to the primary care provider. SCR will focus on patients (a) new to the caseload and (b) not improving as expected under the current treatment plan. SCR may be conducted by phone, video, or in person.

· Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications. Treatment response tracking may be conducted by phone, video, or in person.

· As needed, modify the care plan for patients who are not improving as expected. As needed, facilitate coordination of more intensive or specialized behavioral health care and/or community case management.

· Facilitate patient and family engagement in care and follow up.

· Attend / participate in community-based visits when clinically indicated to support youth and family advocacy efforts (e.g., school visits, CSE meetings, DHS appointments), as needed.

Administration

· Complete documentation in eRecord within 24-72 hours (depending on the task).

o Document progress and treatment recommendations within the primary care electronic health record.

o Maintain a patient caseload registry within the primary care electronic health record to track patient follow-up and clinical outcomes.

· Attend / participate in designated meetings (e.g., SCR, weekly Treatment Team), trainings, and ongoing skill development, as required.

· Help maintain updated care management information related to behavioral health community resources.

· Identify opportunities to improve collaborative care processes and services.

· Share with practice leadership any barriers to meeting patient and family needs.

· Collect and provide reports of activities, as required.

· Performs other duties as assigned.

QUALIFICATIONS

Education

· Master’s degree in Social Work, Mental Health Counseling, Marriage & Family Therapy, or related field.

· Professional licensure is preferred but not required.

Experience

· Prior experience working with children, adolescents, and families in the mental health field.

· Knowledge and ability to provide behavioral health assessments, evaluations, and brief evidenced-based interventions for common behavioral health disorders. Experience with evidence-based techniques such as behavioral activation, problem-solving treatment, medication adherence, and motivational interviewing.

· Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families.

· Dedication to delivering a team-based integrated model of care.

· Demonstration of cultural competence and humility to help ensure that all patients and families feel welcome.

· Excellent communication and team skills, including ability to form strong collaborative interdisciplinary partnerships across care settings.

· Ability to work independently and meet administrative and documentation deadlines.

· Creativity and strong organizational skills.

· Knowledge of community resources.

· Sound computer knowledge and skills including an aptitude for using current health information technology to guide BHCM activities and functions.

· Valid NYS driver’s license and access to reliable transportation that enables fulfillment of position’s travel requirements.

· Adherence to Health Insurance Portability & Accountability Act (HIPAA), The Joint Commission (TJC), Occupational Safety & Health Administration (OSHA), and infection control/safety guidelines, regulations, and policies
