The Safe Options Support (SOS) Care Manager will be a key member of a collaborative, multi-disciplinary team seeking to engage the unhoused population of Monroe County from a trauma informed, person-centered, strengths-based, non-judgmental, autonomous, and barrier free approach. The team’s mission is to is to ensure the greatest level of independence and well- being for homeless and unstably housed individuals who are traditionally marginalized or underserved with compassionate care and connection to needed resources. The SOS team consists of a Team leader, Licensed Clinician and/or Nurse, Care Managers, and Peer Specialists. The team will support program participants through the application of the highly acclaimed, Critical Time Intervention, evidence-based, model of care. Care Manager Position Summary: The position will be embedded within the community with a remote/hybrid component as well. This position is Monday-Friday with rotating telephonic on-call requirements. The primary goal of the Care Manager is to support individuals who are housing deprived to secure permanent housing of their choice. Additionally, SOS team members will provide support for individual’s self-defined vision of personal wellness. Continuing education, supervision, and training to ensure success will be an ongoing part of the Care Manager’s role at Liberty Resources. Care Manager Job Responsibilities: Consistently outreach and build relationships with displaced and unhoused persons; Meet participants where they are physically and behaviorally at known “hang outs” or “Hot Spots” within the community, hospital settings and anywhere applicableMake referrals to necessary care and support services utilizing warm-handoffsWork closely with any existing care team(s) of individuals being servedMonitor engagement in services and status of permanent housing through continued connection to ensure stability, independence and wellbeing in their new communityAdminister assessments related to mental health, addiction, and/or social determinants of health (training is provided)Crisis management and safety planning using the Columbia Protocol and other best practices.Rotating night and weekend shift flexibility. Care Manager Qualifications: Preferred candidates for the Care Manager position would have a Bachelor’s degree or higher, preferable in psychology, social work, sociology, or related field or be a New York State Licensed Practical Nurse (LPN). Past case management work experience will be considered in lieu of Bachelor’s degree Experience providing advocacy with homeless and/or precariously housed populations preferred but not required.Experience as provider and/or recipient of behavioral health, substance use and/or homeless services.Knowledge of:homeless resources within the community;treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff;community based organizations and opportunities for continuing wellness development of participants;intentional peer support models, best practices and methods of implementation.Aware of the various personal and systemic issues that hinder access to care Bi-lingual in Spanish and English preferred. Care Manager Required Skills and Abilities: The successful candidate is required to regularly sit, stand, walk, bend, climb stairs and lift objects of up to 20 lbs. as needed. The candidate is required to possess the ability to understand, support and operate from a strength-based approach, display professionalism in all settings, and recognize the importance and benefits of relationship building and collaborating with other service providers. The ability to solve complex problems using creative solutions and act consistently in a manner that demonstrates dignity and respect for participants is required, as well as the ability to de-escalate participants in times of crisis. A respect for and high level of comfort around people experiencing psychiatric crisis and the ability to utilize critical thinking, problem-solving and de-escalation skills to assist with creating and maintaining an environment that promotes recovery is essential. Sensitivity to cultural differences and ability to incorporate that understanding into service provision is required.
