As a Care Manager within a Health Home program, you'll serve as a vital link in our healthcare system, specializing in managing a diverse patient population with complex medical needs. Your primary responsibility will be to facilitate access to essential services and resources for patients, addressing medical, social, and economic factors that may impact their health outcomes adversely. Utilizing your strong communication and computer skills, you'll effectively coordinate care plans and make referrals to community resources to enhance the overall well-being of your patients.

This is a Direct Hire position with a competitive salary range of $22-$26/hour, accompanied by excellent benefits. The work schedule is full-time, Monday through Friday, typically between 8:00 AM to 5:00 PM.

Job Requirements

• AAS degree or higher in a relevant field is required.
• Previous experience in care management is highly preferred, although recent BSW graduates will also be considered.
• Must possess reliable transportation and a valid driver's license, as there is significant travel involved throughout Monroe County.
