Duration – 13 weeks
Hours – 07:00-15:30, 40 hours per week day shift. every third or 4th weekend. rotating holidays
Job Descriptions:
- Conducting intake assessments with patients and families.
- Ensuring that families feel sufficiently equipped and supported.
- Developing individualized treatment plans that account for physical, financial, and other constraints.
- Adjusting treatment protocols, as needed.
Required Certifications: CT RN License, AHA - BLS, Covid Card
Minimum Experience: 1 year working as a RN Case Manager in a Hospital setting.
