Job Description:

Starting Annual Salary: $38,000

Juvenile Review Board

The JRB Case Manager engages youth aged 10 to 17, arrested or not arrested, considered defiant of school rules, Truant, and involved in delinquent or immoral behavior. The Case Manager contacts the youth’s guardians to assess interest in supportive services and schedules initial screening. Youths are diverted from the court for immoral or delinquent behavior involving an arrest or non-arrest and deemed a low risk for future serious offenses. Youths are identified as displaying defiant school behavior by their school. In both cases, the Case Manager helps the youths accept responsibility for harm to the victim, themselves, and the community, repair the harm and heal through the Balanced and Restorative Justice model (BARJ). The case manager prepares the youth to respond to questions from the volunteers who represents the community and come up with relatable responses to the offense. The Case Manager completes an initial screening for the youths in the (FWSN)Family with Service Needs group or Truancy. The initial screening, scoring of the instruments, and community /school collaboration helps with the development of a unique plan to enhance progress and address the needs of the family. For all of the referrals the case manager will apply a “System of Care” philosophy and ensure the process is client-centered, family driven, and strength based.

JOB SUMMARY: The Case Manager will provide comprehensive case management services to clients and their families/significant others to ensure they have access to comprehensive, community-based services, and that all requirements from referral sources are met.

KEY RESPONSIBILITIES:

  • Focuses on engaging clients from the time of the referral through the entire course of the client’s treatment services.
  • Assists clients in connecting with and gaining eligibility for entitlement programs, including public and/or private insurance programs.
  • Conducts standardized screening for health-related social needs and offers resource and referral services specifically tailored to that client.
  • Provides linkage and navigation assistance for clients and their families/significant others so they can access all relevant community services.
  • Helps clients and their families identify and access other Village clinical services and/or other community resources.
  • Maintains and updates service data to ensure accurate reporting, when required.
  • Develops and maintains collaborative relationships with other community agencies, organizations, and referral sources.
  • Complies with all agency policies and procedures, HIPAA regulations, Joint Commission standards and funding sources requirements including complete, timely, and accurate documentation of all relevant client information as required.
  • Demonstrates cultural competence necessary to serve diverse client population.
  • Participates in a collaborative team approach and engages in ongoing quality improvement efforts to enhance the functioning of the program.
  • Contributes to agency visibility and development of community linkages by representing The Village in community groups or committees, when directed by Supervisor.
  • Performs other duties and responsibilities as assigned by management.


Required Experience:


EDUCATION

  • Bachelor’s degree preferred. Associate degree (or credit equivalent) in Human Services or related field required.

EXPERIENCE

  • 3 years’ experience in a comparable position in a human services agency preferred.
  • Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred.
  • Excellent oral and written skills and ability to communicate comfortably with children, parents, and both primary care and behavioral health professionals.
  • Organized and able to work independently with minimal supervision.
  • Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows environment.
