Job Description:


Starting annual salary $47,000 and $49,500 for bilingual.

JOB SUMMARY

The Targeted Case Manager (TCM) will ensure that clients and their families/significant others have access to comprehensive, community-based services, and that all requirements from referral sources are met.

KEY RESPONSIBILITIES

  • Focus on engaging clients from the time of the referral through the entire course of the client’s treatment services.
  • Ensure that clients are connected to public or private insurance, whenever possible.
  • Conduct standardized screening for health-related social needs and offer resource and referral services specifically tailored to that client.
  • Conduct standardized interviews to collect National Outcomes Measurement System (NOMS) data for CCBHC clients at admission, discharge, and specified intervals during care.
  • Provide linkage and navigation assistance for clients and their families/significant others so they can access all relevant community services.
  • Help clients and their families identify and access other Village clinical services and/or other community resources. Screening will provide the basis for referrals, and navigation services will include advocacy and follow-up. Other Village services may include specialty treatment for children and adults.
  • Demonstrates cultural competence necessary to serve diverse client population
  • Maintain and update service data to ensure accurate reporting, when required.
  • All other duties as assigned.
Required Experience:


EDUCATION

  • Highschool diploma required
  • Bachelor’s degree in the human services field or Bachelor of Social Work preferred with consideration given to candidates with experience in a comparable position in a human services agency

EXPERIENCE

  • Excellent oral and written skills and ability to communicate comfortably with children, parents, and both primary care and behavioral health professionals
  • Organized and able to work independently with minimal supervision.
  • Flexible and able to multi-task.
  • Proficient with word processing and spreadsheet applications. Possess solid general computer skills, including ability to work in a Windows environment. Familiarity with specific applications used by The Village and its referral sources is a plus.
  • Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred
  • May hold the following strongly preferred requirements:
    • Has 3 years’ experience in a comparable position in a human services agency
    • Bi-lingual (English/Spanish)
