Days: Monday - Friday

Hours: 8:30 - 4:30 typical work schedule with some flexibility 

Full Time Benefited Eligible Position

Compensation: $19.50-$23.00/ hour

The Housing Case Manager provides case management and supportive services to individual tenants at Soromundi Commons. Services are delivered using a low barrier, harm reduction, housing-focused, person-centered, trauma-informed, culturally conscious, voluntary service model, which are designed to support residents in maintaining housing and developing the independent living skills necessary for living in the community.

  • Provide a full range of case management and supportive services to a caseload of 24-30 tenants in the Permanent Supportive Housing Program, including DSS participants, meeting with each tenant on his/her caseload a minimum of one time per week;
  • Perform participant’s intake and discharge assessments and develop an individual service and action plan necessary to meet identified participant’s needs, and that will result in increased participant’s functioning, including measurable goals to be achieved;
  • Engage and work in partnership with participants to increase or maintain involvement in learning, motivation to change, employment, health, and social activities;
  • Develop and/or relationships with clinical, healthcare, educational, vocational and other community-based social support services to support and advocate for participants in the community, with increasing levels of independence;
  • Provide assistance with maintaining housing, including interventions with Konover Residential Property Management serving as the landlord; working as part of a team, but understanding each entity has separate missions;
  • Provide individual or group feedback, role-modeling, counseling, instruction and/or assistance with daily living activities, including cooking, cleaning, personal hygiene, and shopping;
  • Provide instruction, advocacy, and ongoing monitoring and support regarding finances, entitlements, and management of resources;
  • Perform continuous assessments and modify interventions, as appropriate; conducting in a timely manner all required HMIS and DHMAS assessments on a periodic and annual basis;
  • Monitor and track participant program participation as established by internal and external reporting standards;
  • Maintain timely and accurate case management logs, Aquity Index and other related participant assessments, and intervention plan records in accordance with agency requirements. Ensure all case notes are entered in HMIS 24-48 hours after any interaction or encounter;
  • Provide transportation to participants as appropriate utilizing personal vehicle for agency business;
  • Maintain a timely timesheet in Paylocity. Includes timely time in and time out, and lunch break;
  • Participate in relevant essential trainings and additional personal development activities to enhance knowledge and understanding of homelessness, counseling, racial justice and economic empowerment;
  • Other duties as assigned.

Requirements

Education:

  • Associate degree or college credits preferred; preferably in behavioral health or related field;
  • Master’s degree desirable but not required; preferably in behavioral health or related field preferred.

Experience:

  • Five years’ experience in behavioral health or related field required or;
  • In lieu of five years’ experience in behavioral health or related field–

- Bachelor’s degree and one to two years’ experience in behavioral health or related field;

- Master’s degree with less than one-year experience in behavioral health or related field.

  • Demonstrated knowledge or ability to work with participants experiencing homelessness, poverty, mental health and substance use issues preferred;
  • Demonstrated knowledge or experience providing residential and community case management services, determining appropriate interventions, and solving practical problems preferred.

Knowledge and Licensure:

  • Bi-lingual Spanish/English preferred.
  • Possession of and ability to maintain valid driver’s license, reliable vehicle, and current registration and insurance required.
