Job Summary:
In person ( FIELD CASE MANAGER)
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. This role requires strong interpersonal skills, the ability to assess client needs, and the capacity to connect clients with appropriate resources and services. Responsibilities:
- Conduct comprehensive assessments of clients' needs, including social, emotional, and financial factors
- Develop individualized care plans in collaboration with clients, outlining goals and objectives
- Connect clients with appropriate community resources, such as childcare services, Section 8 housing, behavioral health providers, and workforce development programs
- Provide ongoing case management services, including regular check-ins and monitoring progress towards goals
- Advocate for clients' rights and ensure they have access to necessary services and supports
- Collaborate with other professionals involved in the client's care, such as healthcare providers, educators, and social workers
- Provide crisis intervention as needed to ensure the safety and well-being of clients
- Maintain accurate and up-to-date documentation of all client interactions and progress
Qualifications:
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Previous experience in case management or a related field is highly desirable
- Knowledge of child welfare systems, Medicare/Medicaid programs, and crisis intervention techniques
- Strong communication skills, both written and verbal
- Ability to work independently as well as part of a team
- Excellent organizational skills with the ability to prioritize tasks effectively
- Proficiency in using computer systems for documentation purposes
