Summary: The Case Manager plays a vital role in providing comprehensive support and assistance to women and children in need. This position involves assessing resident needs, developing individualized service plans, and coordinating access to various resources and programs. the Case Manager serves as an advocate and liaison, working collaboratively with residents and community partners to facilitate positive outcomes and promote self-sufficiency.
Essential duties & Responsibilities:
Assessments: conduct thorough assessments to identify resident strengths, needs, and barriers to self - sufficiency.
Service Planning: develop individualized service plans in collaboration with residents, setting goals and identifying appropriate resources.
Resource Coordination: connect residents with a range of services and resources, including housing assistance, healthcare, employment support, education, and financial assistance.
Advocacy: advocate on behalf of residents to ensure they receive needed services and accommodations from government agencies, nonprofits, and other organizations.
Monitoring and follow-up: monitor residents' progress, conduct regular check-ins, and provide ongoing support and guidance to promote goal achievement and self-sufficiency.
Documentation: maintain accurate and up-to-date case records, progress notes, and documentation in accordance with policies and regulatory requirements.
Collaboration: collaborate with interdisciplinary teams, including social workers, healthcare professionals, educators, and legal advocates, to coordinate services and address residents' needs.
Training & Development: obtain and maintain annual training certifications offered by the Coalition for the Homeless and HMIS system and other trainings.
Case Load: effectively maintain a case load between the range of 30-40 residents.
