JOB TITLE: CASE MANAGER

PROGRAM: Life Is Precious Program

FLSA STATUS: Exempt

EMPLOYMENT STATUS: Full time, 35hrss

REPORTS TO: Program Coordinator, Sr. AVP of Program

JOB SUMMARY

The main projected goal of Case Manager for the Life is Precious program is two-fold: one- to reduce suicidal risk, ideation and attempts by providing robust programming and holistic case management to Latinx girls aged 12 -17. The Case Manager is responsible for providing program coverage, and to provide individual, group and family supportive counseling to the youth and their families. The Case Manager will lead and/or assist with activities and groups in collaboration with other staff; and two - to increase referrals to the program by providing Outreach and Awareness workshops and presentations to the broader community, and by developing, nurturing and strengthening community partnerships with schools, mental health clinics, youth programs, and other stakeholders.

ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES

  1. Liaise to provide outreach, engagement and follow-up with girls referred from emergency departments to assure continuity of mental health treatment and encourage participation in the Life is Precious program.

  2. Provide case management and counseling to connect the clients with services and improve coping and interpersonal skills.

  3. Ability to think outside of the box with creating programs that stimulate the interest and are relative as well as age appropriate for young adults. In addition, activities should expand horizons and provide healing, calming experiences for the clients.

  4. Reinforce vocational exploration, internships and community service to enhance participants’ career planning, learning, personal growth and civic responsibility.

  5. Coordinate trips, celebrations and special events on weekends to bring whole families together and increase the clients’ cultural pride and awareness.

  6. Collaborate with various community agencies, public officials, and organizations.

  7. Assist with all recruitment efforts.

  8. Meet on a regular basis with the Program Coordinator to discuss accomplishments and program related matters.

  9. Represent the program amongst community providers (i.e. schools, churches and community centers) relevant to the program focus and populations.

  10. Document community activities, outings and development involving families

  • Proven commitment to positive youth development principles

  • Proven ability to provide strong therapeutic, social skills and analytical thinking to program matters regarding the Life is Precious Program.

  • Demonstrated skill to communicate effectively in writing, verbally, and to listen actively with a non-judgmental approach. Demonstrated skill in providing outreach and workshops to community partners

  • Thorough knowledge of program development, creativity, and the ability to engage and motivate adolescents.

  • Responsible, organized and efficient, with the ability to work independently

  • Demonstrated ability to perform in a team approach and accomplish multiple assignments simultaneously.

  • Related experience that demonstrates being community minded, patient, creative, flexible, compassionate and culturally competent

  • Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State and City officials, professional and technique groups, clients and the general public.

  • Ability to plan and successfully implement and gather participant related data for programs.

Education and Experience Requirement(s)

BA or BS in human services field; experience working with children and families; Bi-lingual English/Spanish.

The Case Manager for Life is Precious™ is a non-clinical suicide prevention afterschool alternative program for Latina teens ages 12-17. The Case Manager supports teens and families and builds protective factors to prevent suicide attempt and ideation through assisting to provide individual, group and family counseling and provides outreach, referral, and follow-up services. This position offers a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more. The salary range for these positions is from $40,000 to $46,000 a year. The positions are located in the Bronx and Queens.

Areas Of Responsibilities

  • Provide outreach, engagement and follow-up with girls referred to program to assure continuity of mental health treatment and encourage participation in the Life is Precious™ program and assist with client recruitment
  • Provide case management and counseling to connect the clients with services
  • Assist in the creation of age-appropriate programing for young adults that serve to expand horizons and provide healing, calming experiences for the clients
  • Reinforce vocational exploration, internships and community service to enhance participants’ career planning, learning, personal growth and civic responsibility
  • Coordinate trips, celebrations and special events for young adults and their families that increase the clients’ cultural pride and awareness
  • Collaborate with community agencies, public officials, and organizations

Required Job Qualifications Education and Experience Requirement(s):

  • Bachelor’s degree in human services or related field, preferably is social work.
  • Experience working with adolescents and families
  • Bilingual skills in Spanish required

  • Proficiency with MS Office and Google product suites
  • Cultural competency in working with Hispanic adolescents and their immigrant families.
