Description

Comunilfe Inc is looking for experienced Case Managers for its OMH Adult Homes program in Queens.The Case Manager’s main responsibility is to provide case management services for a caseload of clients. The Case Manager prepares comprehensive assessments and service plans. Home visits and Fieldwork are required. This position is eligible generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.
Essential Duties:
  • Ensure that each client is receiving appropriate and effective case management services
  • Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.
  • Conduct face-to-face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
  • Provide assistance in accessing primary medical care; assistance with adherence to medication regimens; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living. 
  • Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs.
  • Provide crisis intervention to clients as needed.
  • Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs.
  • Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services
  • Ensure that clients pay the balance of their rent, telephone and utility bills.
  • Provide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing.
  • Assist clients in moving from hospitals, nursing homes, or other locations into their apartments, and provide transportation when necessary.
  •  Case managers will provide advocacy & referrals to employment programs for training and job placement in the private sector with consultation with clinical supervisors.
  • Assist clients in locating alternative housing options, when a client’s health so requires.
  • Provide and coordinate all supportive services as needed.
  • Collaborate with all providers within and outside of Comunilife involved with the client/family. Coordinate and collaborate with OMH personnel as required.
  • Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.
  • Maintain and complete required documentation including all contacts made with the client and any contact on their behalf.
  • Participate in weekly supervision.
  • Submit a weekly schedule of field visits.
  • Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors.
  • Participate in discharge planning with hospital staff for a hospitalized client(s).
  • Provide escort services as needed to medical or mental health services. 
  •  Provide support and education to the client on budgeting to meet financial obligations.
  • On-going assessments of client's living environment.
  • Perform any other duty as assigned by the clinical supervisor or the Program Director.

Qualifications


Bachelor's Degree required; with a minimum of one to two years fieldwork experience OMH direct work experience. Individual, and group counseling experience required, particularly with mental health illness affected populations and/or substance abuse individuals. Preference is given to persons with experience in Scattered Sites Programs, bi-lingual, bi-cultural in Spanish, a plus.
