Hospice of New York is Hiring a RN Hospice Case Manager Near Long Island, NY
Main Responsibilities: Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Will provide end-of-life care in the Nassau county community homes, IPU and SNF locations.
Requirements:
- Registered Nurse licensed in NY state
- Bilingual in Spanish (not required but highly preferred)
- Must be Covid-19 Vaccinated
- 2 years of RN (hospice, med-surge or home care experience) preferred. Will also consider a new RN graduate who will participate in HONY Mentorship Program.
- Ability to work with limited supervision.
- Strong interpersonal skills.
- Collaborative and team-oriented personality.
- Must have a motor vehicle to reach patients.
- Experience with Electronic Medical Records a plus.
Job Type: Full-time
Pay: $85,000.00 - $105,000.00 per year
