Director – Care Coordination
Purpose:
The purpose of this position is to oversee care coordination programs that connect individuals and families with substance abuse and mental health needs upon their discharge from hospitals and provide support with therapy and care coordination services until linked to long term community providers or consumer needs are resolved. The Director of Care Coordination is accountable for ensuring the delivery of high quality comprehensive and cost-effective clinical services and enhancing overall service area performance in a culturally sensitive and appropriate manner.
