Responsibilities

The Manager of Care Management is responsible for creating safe, healthy environments that support the work of the care management team to improve patient health outcomes, patient engagement, and patient experiences while minimizing cost to patients and families and SHC. The role is influential in creating a professional environment and fostering a culture where interdisciplinary team members are able to apply the SHC FOCUSED TM Model of care and Care Management guiding principles. Provides service and supervision in the practice of Care Management to ensure processes optimize (a) population health, (b) management of care transitions and (c) deployment and adherence to the plan of care. Demonstrates knowledge of, and expertise in, the philosophy and standards of professional Care Management. Assists with the application of these philosophies and standards to diverse populations with special consideration of age and cultural needs, as well as SHC requirements. Excellent customer service skills required. The ability to manage conflicting priorities a must. Empathy, caring and compassion for children with complex medical conditions and their support systems is required. Qualifications

Education

• BSN required, and
• MSN or Master of Social Work preferred.

Certification/Licensure

• RN Illinois License required, or
• Social Worker Illinois License.
• Case Management Certification (CCM, ACM, or ANCC) required. If not currently certified, certification by CCM, ACM, or ANCC will be required within one year of hire, or upon eligibility.

Experience

• 2 years experience in a Healthcare (medical / surgical) setting
• 2 years Care Management
• 2 years Pediatrics experience preferred
• 3 years management experience in a supervisory/leadership role preferred

Other

• Current full vaccination status required

Job Type: Full-time

Pay: $85,000.00 - $125,000.00 per year
