Join the diverse team at our 300 patient IDPH licensed & accredited Home Health Agency. Duties include: Oversee field clinician scheduling, supervise 4 person office intake staff; hands on evaluation and determination of referrals to accept; communicate with referral sources (primarily major hospitals), patients/families, PCP clinics, QA staff, and billing staff; regulatory compliance. Requires minimum 7 years experience as RN, 2 years in hospital discharge case management. ER/ICU experience preferred. Reports to agency administrator and supervisor. Full time in office. Intermittent weekend work with compensatory time off. Strong communication skills and patience needed. Pay commensurate with experience and is salaried. Other benefits negotiable.
