Family Support Specialist

POSITION SUMMARY:

Develops rapport and engages with all family members in their home or community. Provides direct contact and assists in serving as an advocate, service worker, and liaison on behalf of a child and their family who has been referred to the program by Department of Children and Families. Assists families in developing solutions, identifying and connecting to community resources as well as other supports based on the families needs. In conjunction with the Parent Navigator promote permanent connections that the support the family within their community by utilizing a family driven, strength based and community based approach.

Accountable and responsible for the following results:

  • Establish rapport with family members, DCF, schools and other providers.
  • Engage, educate, empower families in a manner appropriate in fostering relationships that lead to family success.
  • Partner with DCF to ensure successful transition into the program.
  • In partnership with the family conduct a needs assessment and develop a strength based and family driven Plan of Care.
  • Facilitate Family Team Meetings consisting of the family, formal and informal supports.
  • Collaborate and develop a shared understanding of respective roles with other team members to ensure efficient collaboration and linkage to community resources and services.
  • Serve as liaison with outside agencies and engage families in the community.
  • Conduct home visits according to program frequency guidelines.
  • Provide array of interventions/supports to strengthen family functioning. Linking multiple services, healthcare providers and community resources to meet the family’s needs.
  • Ensure flow and exchange of information among family members, the team and linked providers/supports.
  • Complete documentation, data input, collect and maintain baseline data and submit information to an electronic state wide system per program guidelines.
  • Complete monthly required engagement surveys and assessments.
  • Attends local collaborative community meetings as appropriate and directed by supervisor.
  • Perform any other department or agency related duties or special projects as directed.
  • Required to use own vehicle on company business and to transport clients as needed
  • Regular and consistent attendance is an essential function of this position

Qualifications


MINIMUM QUALIFICATIONS

  • Bachelor’s degree in Human Servicses from accredited post secondary institution.
  • Minimum one year experience working with children and families in diverse populations.
  • Ability to work a flexible schedule.
  • Sensitivity to diverse cultures and language competencies.
  • Knowledge of behavioral health issues for children and families and of home-based services preferred.
  • Good oral and written communication and documentation skills.
  • Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
  • Ability to prioritize and organize schedules/demands.
  • Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
  • Experience working with families with complex needs preferred.
  • Bi-lingual (English/Spanish) preferred.
