• Collaborate with the VP of Impact, the Data Analyst & Evaluation Specialist, and other key staff to design a trauma-informed health program logic model, including how to measure success. Authentically engage survivor input into program design. Seek to document and communicate evidence of program effectiveness.
• Liaise with key health partners, including Federally Qualified Health and Mental Health Centers, therapists, substance use disorder treatment providers, exercise studios, and other relevant community-based partners. Manage contracts and MOUs. Cultivate new partnerships as needed to meet survivor needs.
• Create a warm and supportive culture of health and health literacy for survivors. Monitor survivor participation and elicit feedback in various options to assess ongoing needs and partnership successes. Collaborate with staff to communicate offerings and encourage participation. Engage staff in offerings as appropriate. Encourage and support survivors through active personal participation as appropriate.
• Facilitate program logistics for survivors to participate in programs through identifying and removing barriers, such as arranging transportation, increasing health literacy, assisting in ing for insurance, etc.
• Proactively communicate across departments to support survivor and program success. Act in accordance with Coburn Place’s organizational values and program principles.
• Contribute to grant seeking or donor engagement efforts for financial support to continue and/or expand the program.

• Bachelor of science in public health, community health, social work, or related field is preferred.
• Experience with trauma-informed health initiatives or health equity is ideal.
• Experience with user-informed health program design and basic user data collection are ideal.
• A history of successful collaboration and program engagement is required.
• A valid Indiana driver’s license and a satisfactory MVR is required for this position.
• A clear DCS history, and clear criminal and sex offender background checks are required.
