We currently have an opening for a Health Care Case Manager - SSVF at our Jones Ave location.

The Health Care Case Manager - SSVF will act as the point of contact of the clinical team dedicated to a specific case load of clients seeking substance abuse and mental health treatment. The CM coordinates services based on daily staffing with other members of the clinical team, updates treatment plans and schedules assessments, transports clients to and from appointments, coordinates for internal and external services based on clinical indicators and provides on-call and after hour support.

Minimum Requirements:

• Bachelor’s degree in a field related to behavioral health OR a combination of education, training, and/or experience
• Valid AZ driver's license with 39 month clear MVR
• 1 year experience working with the GMH/SA population
• 1 year experience in a behavioral health position providing outreach and engagement activities
