This position is responsible for coordinating the social service needs of the maternity program residents and outpatient counseling clients.
Provide intake interviews, orientation process and discharge practices per program policy and procedures. Acquire and maintain pertinent paperwork/records and develop and implement individual service plans. Complete necessary reports, summaries and aftercare plans. Coordinate and arrange social services activities for clients, including but not limited to housing, employment, transportation, food, substance use recovery, childcare and completion of applications.
Complete required billing after shift and obtain client signatures, as appropriate. Record occurrences that happened during each shift in the staff log and record client whereabouts in the client log, when at the Lourdeshouse facility.
Responsible for interactions with each resident. There must be 1 hour per week for individual service planning and addressing social service needs. Participate in “House Meetings” and other client focus conferences.
Become familiar with and develop liaisons with appropriate Human Service to best advocate and make referrals for Lourdeshouse residents and outpatient counseling clients.

Use of hands, wrists and fingers and lifting or moving up to 25 lbs. will be required. Movements such as stooping, crouching, bending, kneeling, and reaching are required. This position requires spending a majority of the workday sitting with occasional standing and walking. The location of the program requires the ability to climb stairs on an infrequent basis if the elevator is not working. Noise and/or level of distractions in the work environment are moderate.
This position will cover casework at Lourdeshouse and Outpatient Counseling, which will require spending time at both locations, which are about 7 minutes’ drive apart. The primary office will be at Lourdeshouse.
Bachelor’s degree required. Preferrable in the human service field. Two years’ experience working in the human services field.
