Catholic Charities of Baltimore is currently seeking a Social Worker who is responsible for the performing case management services which provide for the social and emotional needs of the elders while maintaining current and existing federal, state, local and facility standards related Social Services and assuring that quality of life is maintained for all elders through individual assessments and intervention.

St. Elizabeth Rehabilitation and Nursing Center provides a comprehensive array of individualized rehabilitation and nursing services, including short-term rehabilitation, skilled nursing care, memory care and in-patient hospice care.

The work schedule is Monday - Friday

JOB DUTIES & RESPONSIBILITIES:

• Develops and maintains a professional/clinical relationship with elders and families through regular contact.
• Makes visits to elders and schedules meetings with family members in order to perform necessary services for discharge planning.
• Works closely with members of the healthcare team in the development of the plan of treatment, including discharge plans, based on the psychological, physical, and social work interventions.
• Develops working relationships with leaders in community resources groups, representing the Agency at meetings and on various committees that address the social needs of the elders.
• Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
• Assist in the evaluation and interpretation of social, psychological and emotional needs of the elders/families.
• Maintain regular communication with attending healthcare team, attending physician and other elder care staff members.
• Record and maintain accurate social service progress notes indicating treatment plans, psychological

concerns and/or adjustments to life as required by federal, state, local, and facility standards.

• Assess and work with emotional problems including assisting elders and family members with anxieties and stress caused by illness and admission to the facility, as well as difficulties in coping the need for specialized care and death.
• Perform other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS:

• Bachelors Degree in Social Work from an accredited institution.
• Two to five years experience in a healthcare facility.

REQUIREMENTS & ABILITIES:

• Excellent verbal and written communication skills including active listening.
• Basic knowledge of clerical procedures and systems such as managing Outlook calendar, email, and recordkeeping.
• Ability to act with discretion, confidentiality, and professionalism in all situations.
• Exceptional planning and organizational skills with attention to detail.
• Strong critical thinking and problem-solving skills.
• Ability to prioritize tasks and to delegate activities as appropriate.
• Requires the ability to utilize computer systems and software necessary to perform position functions. Basic Windows PC, web browsing (i.e. Chrome, Internet Explorer, etc.), and Microsoft Outlook skills required. Other Microsoft Office application knowledge, such as Word, Excel PowerPoint, Skype, and OneDrive desired.

 PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

• Light work that includes moving objects up to 20 pounds.
• Assessing the accuracy, neatness, and thoroughness of the work assigned.
• Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with others.
