General Summary of PositionProvides care coordination, psychosocial support, and community outreach to a panel of frail elders receiving home-based primary care in a defined geography. Member of an interdisciplinary team of providers that includes physician, nurse practitioner, and care coordinator who are all responsible for ensuring outcomes aligned with goals of care for their panel of patients.

Primary Duties and Responsibilities

Minimum Qualifications Education

• Master's degree in Social Work required

Experience

• 1-2 years experience required and Home health care preferred

Licenses and Certifications

• LGSW in respective state of employment required and current American Heart Association (AHA) or Red Cross CPR certification required

Knowledge, Skills, and Abilities

• Demonstrated high level problem solving, communication, and independent judgment skills.
• Customer service and community outreach oriented.
