Job SummaryThe Care Coordinator is a member and facilitator of a cross-system team to serve families and their children with challenging behaviors through Wraparound with Intensive Services (WISe). This position works with the child and family to develop and implement individualized care plans that work toward the family’s long-term goals. Services are provided where and when convenient for the family, including home and community based. The Care Coordinator will assist as a member of the team to implement positive behavioral supports in the home. Care Coordinators must be skilled in facilitation, managing team conflict, engaging cross-system team members and communicating with diverse stakeholders and natural supports.

Job Duties/ResponsibilitiesTo perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below.   1. Provide intake observations of the child in all life areas including home, school and community to develop a cross-system plan of care.
  2. Assist in completing family and behavioral health assessments with child and parent, including CANS (child adolescent strengths and needs).
  3. Participate in decision making for high-risk families, contributing to the development and implementation of an individualized treatment plan that outlines supervision and assessment of current mental health and safety needs.
  4. As part of the clinical team, assess the family’s need for community resources to ensure safely and recovery-oriented treatment.
  5. Provide and create a supportive environment for parents and caregivers by encouraging hope in, and control over, their own lives as well as their children’s lives.
  6. Promote self-determination, empowerment, personal responsibility for recovery and wellness, and assist parent/caregivers to advocate for themselves and their children.
  7. Partner with clinician, family partner and other members of the team in creating a wellness plan that empowers the family to reach their goals.
  8. Collaborate with the treatment team to promote a team culture in which decision-making in treatment and the cross-system’s plan ensures the child and family needs are addressed.
  9. Act as a partner to the family and as a liaison with community providers, school representatives, legal system, and support systems.
  10. Utilize Motivational Interviewing techniques to ensure the cross-system plan of care maintains a focus on strengths, needs and capacities of the child and family.
  11. Accurately complete documentation per required timeframes and recorded in the Electronic Health Record.
  12. Update the cross-system plan of care after each monthly meeting in a strength-based manner.
  13. Provide required reporting for alleged abuse or neglect, parental relapse with drugs or alcohol, safety plan not followed, new safety issues identified, severe behavioral incidents, running away, or situation requiring medical attention or hospitalization.
  14. Lead monthly CFT (Child and Family Team) meetings.
  15. Participate in weekly meetings with the families help them problem solve and develop an effect time management plan.
  16. Provide families with time to prepare for court hearings, FTDM (family team decision meetings) and other dependency related appointments. Attend appointments to support families and advocate for the family’s needs.
  17. Complete HMIS housing assessments and assist the families in the search for permanent, stable housing.

Other Relevant Duties

  1. Carry out shift responsibilities (as assigned) during work hours designated at Rising Strong; staff other program areas as needed to provide support to the program.
  2. Actively participates in staff meetings and multi-disciplinary meetings with other team members.
  3. Actively participate in team and/or individual supervision as stated in agency policy.
  4. When subpoenaed or otherwise called upon, appear in court to testify regarding families’ involvement in the program in a professional manner.
  5. As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect
  6. Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI). Per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  7. Will regularly utilize all forms of communication (email, text, phone, etc.) and database practice necessary for this position and as directed by your supervisor.
  8. Adheres to the tenets of Catholic Social Teaching and Catholic Doctrine.
  9. Performs related functions necessary to support the mission and core values of Catholic Charities.

Job QualificationsTo perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job. Education/Experience:to perform this job successfully, an individual must have a bachelor’s degree in social services, psychology, or education field and a minimum of two years of related experience and/or training or an equivalent combination of education and experience. Experience working with behavioral health services is desired.Required Skills:

  • Ability to work flexible schedules to work with family’s needs
  • Operate from a strengths-based perspective
  • Possess good written and verbal communication skills and excellent documentation skills.
  • Ability to work independently and as a team member
  • Working knowledge of community resources
  • Ability to represent the agency in a professional manner within the community and develop collaborative community relationships.
  • Basic computer proficiency

Preferred Skills:

  • Knowledge of Spokane City and Spokane County community resources.
  • Experience working with issues of homelessness, substance abuse/addiction, mental health and/or poverty preferred.
  • At least two years successful experience in providing case management services and direct client services.
  • Demonstrated experience working with culturally diverse populations and/or demonstrated life experience which qualitatively improves service delivery to Rising Strong families (i.e., diverse health perspectives, cultural and family style approaches).
Certificates/Licenses: To perform this job successfully, an individual must:
  • Possess valid WA State driver’s license and clean driving abstract.
  • Successfully pass background check applicable to position.
  • Eligible for Agency-Affiliated Counselor registration required
