American Behavioral Health Systems is Hiring a Health Care Coordinator Near Spokane, WA

Job Summary:

Our HCCs assist and monitor the Lead and Care Team staff and building security and maintain appropriate order and safety for all clients. Act as a liaison between the ARNP consultants and coordinate care services for clients. To respond to client needs and concerns.

Qualifications:

High School diploma or equivalent and two years experience related to job description. Some college preferred. Must have a genuine concern for people, ability to relate to people, be reliable, and conscientious.

Physical Demands:

Must be able to sit, stand, and walk. Must be able to lift up to 25 pounds. Ability to use hands to fingers.

Work Environment:

Moderate noise (business office with computers, printers, light traffic).

ESENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be

assigned.

1. Assists supervisor as needed.

2. Coordinates care services with all health care providers and consultants.

3. Maintains client medical information to assure completion and accuracy.

4. Monitors client population for communicable disease outbreaks.

5. Assist with training of new employees in all aspects of medication and client care.

6. Assure security of client files and other records.

7. Maintain such records as required by supervisor.

8. Patrol building as necessary or required.

9. Be available to talk to clients if necessary.

10. Monitor client’s self-administration of medications as per WACs and ABHS policies and procedures.

11. Be knowledgeable regarding safe policy and procedures related to position.

12. Properly inventory client’s property on arrival and discharge.

13. Properly inventory any new property each time a client returns from a pass.

14. Observe specimen collection and provide documentation for urinalysis (UAs), and ensure that all care staff do the same.

15. Be knowledgeable regarding safe policy and procedures related to position.

16. Keep accurate, legible written log of events that take place during the course of your shift.

17. Ensures that clients to do not leave the building without an appropriate pass.

18. Through the use of progress notes, keep counselors informed daily of any activities concerning clients on their case load.

19. Check all packages being brought into the building by anyone, and ensure that the care staff do the same.

20. Check in any medication as soon as possible after they enter the building, and ensure that all care staff do the same.

21. Enforce rules and regulations related to resident activities and behavior.

22. Drive the Agency Van when needed (if pre-approved).

23. Provide support and assistance if qualified, whenever and wherever needed to assure quality services for agency consumers.

24. Ability to take written and verbal orders.

25. Performs other related duties as assigned.

26. Responds to the directives of the supervisor and management staff in a professional and cooperative manner.

27. Assures that all ABHS’ clients’ needs are met with dignity and respect and in a timely manner.

28. Makes decisions based on the welfare of the client, the success of the team, and the quality and integrity of the program.

29. Understands and facilitates federal confidentiality laws, 42 CFR, and protects client confidentiality/anonymity, and assures no information is shared without proper authorization.

30. Ensures compliance to contractual obligations, ABHS policies, and State statues (WACs).

31. HCC's will be required to obtain an Agency Affiliated Credential through the Department of Health.

32. Duties as assigned.
