CAP is excited to announce that we are hiring an experienced Clinical Case Manager. The Clinical Case Manager works with the staff and management of Cascade AIDS Project (CAP) to provide high-quality, coordinated, strengths-based social services consistent with the agency's mission. The individual works on Cascade AIDS Project's SW Washington team to guide up to 70 medium and high barrier and/or Medicaid-eligible individuals living with HIV through the continuum of HIV-related and other relevant systems of care. This requires maintaining an awareness of cultural issues and developing effective relationships with a diverse client population.

Responsibilities include:

  • Screening clients for psychosocial needs
  • Developing goal plans
  • Implementing effective interventions and monitoring client progress
  • Assisting clients with applications for insurance, public assistance benefits, etc.
  • Monitoring the process from initiation through formal decision
  • Providing information and referral to a wide range of community services
  • Providing advocacy within multiple settings including educating clients to self-advocate when appropriate.
  • Providing problem solving interventions as needed to assist client's completion of their goals.
  • Working closely with providers and inter-disciplinary team to facilitate client care and resolve short-term needs.

The Clinical Case Manager will support CAP's efforts to increase staff knowledge and competency around mental health challenges as related to HIV and barriers to medical engagement and will provide consultation and support to all members of the SW Washington team who are working with PLWH with a co-occurring mental health diagnosis. The person in this position is stationed in CAP's Vancouver office and will travel throughout the Clark, Cowlitz, Skamania and Multnomah County service area for meetings and client home visits.

NOTE: Occasional evening and weekend work, a state-issued driver's license and access to a personal vehicle are required. This is a non-management, union-represented position. The compensation, benefits, and conditions of work for this position are collectively bargained. 

COMPENSATION: $60,403.20 annually

MINIMUM QUALIFICATIONS:

  • Master's Degree in field of Mental Health (Social Work, Psychology, Counseling) OR Master's Degree in related field with a minimum of two years providing case management (or similar) services to people with high barriers to care, high needs, etc.
  • Prior experience working with persons with severe and persistent mental health challenges.
  • Prior experience providing case management (or similar) services.

PREFERRED QUALIFICATIONS:

  • Demonstrated computer proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the
  • Excellent written and oral communication skills
  • Successful experience working with ethnic, racial, economic and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction.
  • Demonstrated ability to effectively collaborate with community stakeholders.
  • Good organizational and time management skills
  • Ability to work independently with accountability and exercise sound judgment, discretion and professionalism at all times.
  • Ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to: engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities that we serve
  • Valid driver's license and access to a reliable vehicle
  • Available to work occasional evenings and weekends.
  • Professional Licensure or Certification (LCSW, LPC, CADC)
  • Verbal and written fluency in English and Spanish
  • Knowledge of community resources in Clark and Skamania Counties
  • Previous experience working with people with HIV.
  • Previous work experience with consumer health insurance and/or knowledge of Washington State health insurance options

EMPLOYEE BENEFITS:

CAP is proud to offer a comprehensive benefits package for eligible employees.

Any non-temporary full-time or part-time employee (working .5FTE or greater) is considered eligible for the benefits. Benefits may be adjusted based on FTE or hours worked. The below benefits are for 1.0 FTE or 40 hours a week.

Vacation Leave Other

  • 16 days of paid vacation (128 hours) each year for the first two years of employment
  • 21 days of paid vacation (168 hours) each year of employment beginning in year three
