Service Request and Referral Case Resource Manager

The Developmental Disabilities Administration within Region 3 of the Developmental Disabilities Administration (DDA), has a non-permanent opening for a Service Request and Referral Case Resource Manager. The DDA Region 3 office is inVancouver, WA.
The DDA in Region 3 provides an advanced level of social services and specialized case and/or resource management for people with developmental disabilities and their families. this role adheres to federal and state laws and regulations, department/administration policies, and procedures governing work tasks.
This position support DDA eligible clients without current services from DDA. They provide an advanced level of social services and specialized case and resource management for individuals to further the mission of Transforming Lives.
Some of what you will do

  • Identify support needs of clients of Developmental Disabilities Administration (DDA) and make referral for appropriate services
  • Communicate with clients ad their families, to discuss their service needs, and to answer questions they have
  • Schedule and conduct in-person, face-to-face home visits with clients and families including regional travel
  • Complete and revise client assessments and service plans in CARE within established timelines as required for caseload and according to WAC and send appropriate notices to client and representative
  • Request Waiver Enrollment Requests (WERs), Exceptions to Rule (ETRs) and Prior
    Approvals (PAs) timely and per requirements
  • Refer individuals to appropriate DDA services, other department services, and community programs/resources as needed or requested
  • Identify clients or families in crisis and make referrals as needed to appropriate authorities, department/administration services, state plan services, or local community resources
  • Collaborate with other agencies and professionals as needed
  • Represent DDA and participate in community and agency meetings as required, demonstrating cooperative, professional communication skills.
  • Identify serious and emergent incidents and report per administration policy and mandated reporting requirements
We are looking for professionals who have
  • Bachelor’s degree in social sciences, social services, human services, behavioral sciences, or an allied field
AND
  • Two years of experience providing social services to people with developmental disabilities
Note: Graduate training in social sciences, social services, human services, behavioral sciences, or an allied field will substitute year for year, for one year of experience providing social services to individuals with developmental disabilities.
OR
  • Satisfactory completion of 12 months as a Case Resource Manager Trainee
Preferred knowledge, skills, and abilities
  • Knowledge of Developmental Disabilities RCWs, WACS, and policies/procedures pertaining to duties of position
  • Possess a high degree of communication skills (listening, observing, communicating, interviewing, understanding, confronting, engaging in problem-solving, and resolving conflict)
  • Compose professional, clear, grammatically correct correspondence.
  • Organize the work environment and prioritize work assignments to complete them in a timely and accurately
  • Listen, clarify, and follow directives, especially the ability to absorb new information and act on it
  • Possess basic computer skills, including use of e-mail, basic software (e.g., word processing, spreadsheets, etc.), system applications (e.g., CARE) and internet access
  • Demonstrate sensitivity to cultural diversity
  • Willing and able to travel as required
