The Patient Navigator Manager will support and implement clinical navigation systems to ensure that daily operations of the Emergency Room patients w/ Mental Health and Substance Use Disorders are supported, and linked to the appropriate Level of Care post discharge. This candidate will lead by example to foster a therapeutic, compassionate team based approach; ultimately ensuring that Bergen New Bridge Medical Center provides excellent treatment onsite; as well as becomes the innovative Leader in “quality of care in between the care.” This Leader will work with the Emergency Room multidisciplinary teams, Recovery/Peer Support, Admission Department and all Service Lines to oversee and assist in screenings, follow-up, case management and prior-authorizations to ensure that our Quality Improvement Program (Q.I.P.) is well positioned for success. Additionally, under the direction of the Q.I.P. Coordinator, the candidate will work closely with Finance and Data Analytics, to act as a Hospital Liaison by communicating regularly with community partners to ensure that all external follow-up/referrals are being captured according to Q.I.P. NJ guidelines. This candidate will also act as a patient and family advocate with a strong focus on patient experience; as well as train, educate and implement the Trauma Informed Care model into the Emergency Room Department.

Qualifications:

• Must have a CADC or pursuing professional licensure in the behavioral health field
• Must possess the knowledge, skills and experience necessary to completely perform case management activities with a minimum of three (3-5) years demonstrated experience working with high need individuals with substance use and mental health disorders.
• Bi-Lingual Spanish preferred
• Ability to work effectively with people of diverse cultural/economic/racial/religious backgrounds, sexual and gender orientation.
• Well organized, with excellent oral and written communication skills.
• Must be willing to accept on-call responsibilities
• Own vehicle for self-transportation
• Good computer skills – Microsoft Office Applications
