Position Description
40-hour Social work position for licensed MSW to provide a range of clinical services and case management to children, adolescents and their families coping with acute and chronic conditions in the Metabolism Program.
This Metabolism Clinical Social Worker will be responsible for:

• Performing biopsychosocial and risk assessments, developing and implementing interventions and treatment plans
• Providing advocacy and complex resource coordination
• Providing psychosocial consultation to the medical team and planning and implementing special events and programs aimed at providing psychosocial education to patients/families and connection to social support.

To qualify, you must have:

• Master’s Degree in Social Work, and LCSW or LICSW is required.
• Experience in medical environment or with chronic illness is preferred
• Knowledge of child development, excellent communication skills, flexibility and ability to work in fast-paced environment.
