Boston Children's Hospital is Hiring a Clinical Social Worker, MSW -Primary Care Near Boston, MA

This Social Work Care Coordinator will work directly with an interdisciplinary behavioral health team in primary care, helping to support various innovative behavioral health initiatives. This social work care coordinator will have also an opportunity to participate in an ongoing multi-site learning collaborative as well as population management related work for this specific patient population.
This Social Work Care Coordinator will be responsible for:
  • Working with patients coming out of higher levels of psychiatric care/ crisis/ and their families to ensure that all appropriate follow up care is delivered.
  • Monitoring, assessing, and intervening as needed for rising or acute risk in the behavioral health patients and effectively communicate and help to coordinate treatment plans.
  • Close collaboration with members of the interdisciplinary behavioral health team, medical providers and community-based providers.
  • Communication, documentation of all plans and conversations with patients and families, and assisting with determining barriers to care.
  • Assisting with ambulatory safety net program design and implementation
To qualify, you must have:
  • Master’s Degree in Social Work and LCSW is required. LICSW is preferred
  • Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
  • Experience with social services agencies, understanding of social determinants of health and trauma ideal.
  • Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!
