Summary
The Case Manager is responsible for providing case management services, individual counseling/therapy, prompt intervention, assessment, treatment planning and implementation, as well as monitoring services for an assigned caseload comprised of mentally ill and developmentally challenged individuals. Additional duties include outreach, medication monitoring, medication training and electronic records keeping in compliance with agency policy and procedures.
