Summary

The Case Manager is responsible for providing case management services, individual counseling/therapy, prompt intervention, assessment, treatment planning and implementation, as well as monitoring services for an assigned caseload comprised of mentally ill and developmentally challenged individuals. Additional duties include outreach, medication monitoring, medication training and electronic records keeping in compliance with agency policy and procedures.

Role and Responsibilities

• Provide case management services such as referral, linkage, advocacy, monitoring, outreach, aftercare and support to assigned consumers.
• Conduct individual counseling/therapy as stipulated in individual service plan.
• Develop the following service documents for developmentally challenged consumers annually:

1. Inventory of Client and Agency Plan (ICAP).
2. Implementation Strategy based upon Discovery Tool and Person-Centered Plan (PCP) provided by PAS agency.
3. Annual Staffing as well as Annual Physical, Dental and Vision Examinations.
4. Ensure that there is a valid Psychological Evaluation Report in consumers’ records (updated every 5 years).

• Collaborate with consumer’s Developmental Training (DT) primary staff to ensure continuity of care.
• Develop and implement treatment plans and conduct assessments such as LOCUS & Mental Health Assessment.
• Complete all required case documentations, billing and correspondences within the given time frame.
• Provide consistent outreach to consumers’ homes via home visits, phone calls, letters etc, in accordance with agency/program guidelines.
• Maintain regular contact with authorized significant family members and collaterals of consumers.
• Monitor consumers’ medication compliance and facilitate consumers’ cooperation in keeping scheduled psychiatric and primary care physicians’ appointments.
• Attend regular scheduled supervision sessions to discuss current caseload activities for staff development and training and to be provided with needed individual support to enhance work effectiveness.
• Interface with community services, treatment programs and natural support systems on behalf of the consumer to ensure that the consumers’ needs are met efficiently and effectively.
• Monitor consumers for the purpose of minimizing psychiatric hospitalization and ensure follow up when hospitalized.
• Participate in all agency and program meetings, staff development and other training provided by the agency.
• Maintain a caseload of 20 to 35 consumers according to agency procedures.

Qualifications

• Bachelor’s degree in Psychology, Social Work or other mental health related fields.
• Must possess good written and verbal communication skills.
• One year experience working with developmentally challenged individuals.
• Direct Support Professional (DSP) and/or Qualified Intellectual Development Professional (QIDP) is a plus but not required.
