Case Manager
La Rabida Children’s Hospital is hiring a full-time Case Manager for our Chicago Child Trauma Center (CCTC) and Continuity of Care Center (CCC) in Chicago, IL.

Job Summary:
Provides trauma-informed specialized case management services for children and families served by the La Rabida Chicago Child Trauma Center (CCTC). Supports the delivery of effective trauma-focused assessment and treatment by assisting with removing barriers and engaging families in the use of community resources (e.g., addressing obstacles such as inaccessibility of safe housing, transportation, food, educational support, and referrals for additional specialty therapeutic services). Develops and maintains relationships with resource and referral sources for children and families served by the CCTC. Assists in maintaining the CCTC trauma-focused service wait list, with duties including building relationships with foster care agencies and caseworkers, efficiently procuring relevant paperwork for effective treatment, and providing outreach and reminder calls to families. May also engage in other supportive activities for the CCTC and may have opportunities to develop additional clinical skills, such as engaging in clinically focused trainings and assisting with therapeutic groups.
The CCC Case Manager provides support services for children who receive psychiatry care, who have had DCFS investigations/are Youth in Care, to support placement stabilization, promote family reunification, and ensure the safety and well-being of children referred for services.
Essential Trauma Therapist Functions:
Clinical Case Management Responsibilities (70%):
Develops and maintains current relationships with resource and referral sources for children and families served by the CCTC, including referrals for additional specialty therapeutic services and referrals for basic needs
Holds regular office hours to facilitate connecting children and families served by the CCTC with community resources
Works at least 1 evening per week
Conduct overflow and/or Spanish (if bi-lingual) intake screenings for families referred to the CCTC
Work collaboratively with the CCC psychiatrist
Collaborate with outside providers involved in the care of patients to promote continuity of care, including but not limited to SASS, DCFS, hospitals, therapists, schools and case workers as clinically indicated
• Coordinate with scheduling dept. to arrange initial and follow up appointments for patients and families
Coordinate with scheduling dept, to reschedule appointments as is necessary
Administrative Responsibilities (25%):
Obtains relevant clinical and administrative paperwork for CCC Youth in Care on the wait list
Assists clinical/program director with managing the wait list
Assists clinical/program director with data collection as needed
Maintain CCC patient charts
Maintain medical consents for treatment (DCFS referring provider will provide these for Youth in Care), for intact families the Case Manager will complete the form with the family and submit for authorization prior to services being rendered
Collect, submit and maintain authorizations for use of medicines
Provide DCFS caseworkers with monthly/quarterly reports
Distribute client satisfaction survey when clients have completed treatment and submit them on a quarterly/annual basis
Additional Responsibilities (5%):
Maintains accurate and up-to-date records and reports in accordance with BSD standards
May have opportunities to engage in clinically focused trainings and may assist with therapeutic groups or providing individual therapy as appropriate
What we need from you:
BSW/MSW Degree required
Training and experience with children and families in a clinical case management role required
3 years of relevant experience with this population highly desirable
Experience with children who have been abused and/or neglected highly desirable. Experience providing case management services required
