Responsibilities
A Housing Case Manager contributes to AHF by providing services to HIV-positive adults who are homeless or otherwise in need by assisting them in finding a safe and affordable place to live. The HCM must complete intake, assessments, service plans, and progress assessments; enroll eligible clients into programs in a timely fashion; and collaboratively develop individual goal-based service plans with clients. Facilitate regular progress meetings and make amendments when needed. Advocate on behalf of clients to ensure they receive needed services. Find apartments for clients when required, establish, and maintain relationships with landlords or property managers, and facilitate the execution of leases. Assist participants with moving and obtaining furniture, home supplies, and other needs. Identify strategies to address any short-term or chronic issues, such as substance use or mental illness, which might compromise the achievement of goals. As a housing case manager, you support clients emotionally while helping them plan their finances and life goals through a structured program. A bachelor's degree in psychology, social work, or a related social service field is required, and 1 – 2 years of case management experience is needed. 1 – 2 years of experience working with individuals experiencing homelessness and individuals with a history of trauma is preferred. Other preferred qualifications include previous work experience with low-income families, mental health services, and HIV Positive individuals. Other essential qualifications include computer proficiency, experience with administrative duties, organizational skills, and excellent verbal and written communication skills. Networking and resource-building skills are vital for housing case management. One must demonstrate a high-level understanding of programs and benefits in their communities on the city, state, or national level and a strong desire to connect clients to these resources.

Licenses/ Certifications: A Bachelor's degree in psychology, social work, or a related social service field is required, and 1 – 2 years of case management experience is needed.
