Eco-Life Employment Group is Hiring a Community Healthcare Worker Near Houston, TX

Overview:

The Mental Health Navigator (MHN) is a key member of the Project H.O.P.E. treatment team. They serve as the initial point of contact for individuals interested in receiving treatment through Project H.O.P.E. They assist clients in remaining engaged in treatment and help clients overcome barriers to recovery through social support navigation, referrals, supporting treatment goals, and relapse prevention efforts. Additionally MHNs will help clients access mental health care and primary care in the community as recommended.

DUTIES AND RESPONSIBILITIES

  • Provide timely, responsive care coordination to individuals interested in mental health treatment through project hope
  • Conducts initial screening calls with potential clients
  • Provides referrals and care navigation support for clients needing primary care and/or mental health treatment in the community
  • Document all client interactions accurately and in a timely manner using the patient registry and electronic medical record (EMR) as appropriate
  • Engage in weekly Project H.O.P.E. team meetings to evaluate clients progression through treatment and identify opportunities for further support
  • Liaison with Bread of Life warehouse team to provide supplies as needed to clients through established BOL procedures
  • Advocate for clients by building trusting relationships to encourage independence and long-term success.
  • Build positive and trusting relationships with community and organization members
  • Effectively manage time to connect with clients, locate resources, document encounters, and update client registry as needed
  • Works as part of the care team to provide community and behavioral health resources as needed
  • Deliver services with dignity, respect, and effectiveness
  • Actively work to mitigate stigma regarding mental health treatment in all client interactions
  • Participate in team training sessions and maintain appropriate continuing education for certification (or licensure, as appropriate)
  • Other duties as assigned

REQUIRED SKILLS

  • Community health worker certification (active)
  • Proficient in Spanish (preferred)
  • Lived experience and/or work experience in social support or healthcare settings (preferred)
  • Strong relational, interpersonal, organizational, and communication skills
