The Baltimore Medical System Anticoagulation Program Manager plays a crucial role in ensuring safe and effective medication therapy for patients receiving anticoagulation treatment. They collaborate with healthcare teams to optimize therapeutic outcomes and minimize adverse effects related to anticoagulant medications. This position requires a deep understanding of anticoagulation therapy, effective communication skills, and a commitment to patient care and safety. This position is responsible for managing all anticoagulation patients within Baltimore Medical System.

Clinical pharmacists ensure safe, effective, and cost-efficient medication therapy is implemented for all patients in their care and monitor care appropriately. This includes preparing medications, participating in the drug distribution process if needed, monitoring drug therapy, providing drug information to patients and providers, providing clinical pharmacy services, solving medication related problems for individual patients as well as in the medication use process, and supervise daily workflow to accomplish daily tasks allotted to the pharmacist. Clinical pharmacists participate in quality improvement projects to improve clinical care or pharmacy operations.
